As a condition of your "Pre-Approval" to continue receiving financial aid, you must develop an Academic Plan in conjunction with your GAC Academic Adviser

Bring the following documents to this meeting:
- Financial Aid Student Referral Form (Yellow Form)

Supporting documentation (e.g., doctor's notes, police and accident reports, etc.) that demonstrates how medical, legal or personal issues, if any, have affected your school work

Your Reinstatement Contract from the Office of Academic Standards (OAS, if applicable)
You must have all of the above items before you are eligible to meet with your GAC adviser!
