If you have received honoraria or travel reimbursements during the stated period, please fill out this
Honoraria Report Form and submit it to the Office of the Dean for Faculty and Staff Relations no later than May 9, 2008 (in hard copy - not by e-mail). As the College Ethics Officer, it is my responsibility to review all honoraria reports and to submit the list of honoraria to the Commission on Public Integrity by the set deadline of June 1, 2008.
